Case Planning Meetings is the Catholic Children’s Aid Society of Hamilton’s family conferencing model. A Case Planning Meeting provides the opportunity for families, children and other supports to the family to be directly involved in the development of the case plan. The case plan identifies the goals and tasks to promote the safety, well-being and permanency of children and youth.
